If you have not already done so, now is your chance to join the National Middle School Association and/or the Oregon Middle Level Association! Below you will find two options for joining. Follow the instructions in the first section and you will become a member both of the national and the state organizations. Follow the instructions in the second section to join only OMLA. Do not do both.

The form for joining OMLA alone allows for individual, team, or institutional memberships. Read the forms for further information on the benefits of team or institutional memberships as well as costs.

Before you begin: Do you have at least Version 5.1 of Acrobat Reader that supports filling in forms? If not, either click on the Acrobat logo and follow their instructions to install the free Acrobat Reader or print the form and then fill it in by hand. (You must have some version of Acrobat Reader to complete this process).

To join both the Oregon Middle Level Association (OMLA) and the National Middle School Association (NMSA):

You can join using NMSA's secure website by clicking HERE, or

1) Click on to download the OMLA registration form (opens in a new Acrobat Window),
2) fill in the information on the form using your mouse and keyboard,
3) print the form using the printer icon in the Acrobat tool bar,
4 ) attach a check for the amount indicated on the form,
5 ) and mail it to:

To join the Oregon Middle Level Association (OMLA) only:

Please Note: If you have completed and sent in the form in the first section above, you are already an individual member of OMLA. You do not need to join OMLA again. If your school is enrolling a team or the entire institution do not include yourself on the list.

The OMLA has chosen not to get involved with moving money via the internet. Instead you have two options for joining OMLA. One is to send an enrollment form and a check to OMLA via U.S. mail. The other is to send your enrollment information to us via the internet and then be billed.

To join the Oregon Middle Level Association and send payment with your enrollment form via U.S. mail:

1) Click on to download the OMLA registration form (opens in a new Acrobat Window),
2) fill in the information on the form using your mouse and keyboard,
3) print the form using the printer icon in the Acrobat tool bar,
4 ) attach a check for the amount indicated on the form,
5 ) and mail it to:

To join the Oregon Middle Level Association via the internet and be billed:

1) Click on to open the enrollment form in a new window,
2) fill in the form
3) click the submit button at the bottom of the form,
4) wait for a bill to be mailed to you,
